Important Medication Information from the Health Office

Dear Parents,

In order for the health office to administer medications for your child in school, we need a doctor’s order.  On that order there must be 6 things:

  1. Name of student
  2. Name of medication
  3. Dose of medication
  4. Frequency of medication
  5. Physician signature
  6. Parent signature

Clinic visit summaries (such as Tria) with suggestions for medications in school are NOT adequate. 

This medication must be brought to the health office in its original bottle, (no snack size baggies). 

This is NOT a new policy.  This has been in place for years and years. 

Once your child is in the 7th grade, he/she may take over the counter type medications with a parent signed consent form on file with the health office.  Please go here, Lake Harriet’s Health service web page for more information.
