A Notice Regarding Immunizations


Dear Parents,

If you recently received a letter from the health office (either @ conferences or via backpack express) about your child being out of compliance with immunizations, know that he or she will be excluded from school on January 6th if this has not been addressed. This is for both Upper and Lower schools. If you have any questions feel free to call the health office at 668-3315 (Upper) or 668-3222 (Lower).

 

To Parent/Guardian of a 7th Grade Student Out of Compliance with Immunization Law:

Minnesota law requires that in addition to the shots required in other grades, all students entering 7th grade must have received vaccination against Hepatitis B (a series of 3 shots), a total of 2 doses of varicella (unless they have had chicken pox), a total of 2 doses of MMR (Measles, Mumps, Rubella) vaccine and a Td/Tdap (Tetanus, Diphtheria) booster unless they claim an exemption. If the student should not have shots for medical reasons or because of the conscientiously held beliefs of the parents, the school must have written proof of exemption. Exemption forms can be obtained from the school health office or from www.mpls.k12.mn.us (go to Departments, then Health Services, and look for Immunizations Needed for School)..

 

To Parent/Guardian of Kindergarten Student Missing Shots:

Minnesota law requires that all students entering kindergarten must have received vaccination against diphtheria, pertussis, tetanus (DTP), polio, measles, mumps, rubella (MMR), hepatitis B, and varicella (chicken pox) unless they claim an exemption. If the student should not have shots for medical reasons or because of the conscientiously held beliefs of the parents, the school must have written proof of exemption. Exemption forms can be obtained from the school health office or from www.mpls.k12.mn.us (go to Departments, then Health Services, and look for Immunizations Needed for School).

Reminder- Minneapolis Public Schools operates under the licensure, authority and guidance of Hennepin County Department of Epidemiology and the MN Department of Health. Our district’s STRICT 24 hour rule regarding fever, vomiting and diarrhea comes from these public health institutions and we are required to comply with it. If your child has a fever, or has vomited, or has diarrhea, then we need a 24 hour window of being fever free, (without the help of fever reducing medications such as Tylenol), vomit, and diarrhea free.
