At Home Covid Test Kits for Middle Schoolers

The state has expanded its mandate around optional saliva testing to include students in middle school on a bi-weekly basis. If you would like an At-Home Saliva Test from the school, please complete the form below. Test kits will be received on on Friday, May 28. Students may return test kits to the main office of Upper Campus on Tuesday, June 1 and it will be dropped off at a UPS site by school staff before noon. Or you may drop off at UPS site over the weekend.
