PTA Check Request Form
This form must be submitted to receive reimbursement from the PTA. Forms must be received by June 5 to receive reimbursement.  There are two ways to submit the form:
 
1) The form may be submitted electronically by clicking here.
 
2) A paper copy can be viewed/printed by clicking on the link below.  
 
Send the form, plus receipts/invoices associated with the request to the PTA Treasurer (treasurerlh@gmail.com) or drop off in the PTA or PTA Treasurer's mail box at the Lower or Upper Campus.
